Statement of Self-Pay
Cash, Money Order or Check made out to Richard Bishop or Sherry Bishop. The Check would be cashed that day to assure it is good. RSB Nursing Assistant Training Program does not accept Debit or Credit Cards.
Documents needed on the first day of class is Government Issued, DL (Driver’s License) or ID (Identification Card) with your picture, Your SS (Social Security Card) for Coping purposes.
By the first day of Clinical Instruction (In the Nursing Home), proof of the results of a TB Skin Test.
